Transitions in care can be hard and could have bad consequences. After a patient moves in the hospital to his house, he has an obligation for his health. They have support and aid in areas an ill patient will face not in the hospital, for example remembering medication, making healthy choices, and completing rehabilitation activities. An effective transition in care requires much effort and care, plus a care transition program may help ease the stress and responsibility medical providers and patients face while they leave a medical facility.
